Please print and fill out the attached new patient paperwork prior to your first appointment. If you do not have access to a printer, you may fill it out in the office, but please arrive 15 minutes early to complete the forms.

The patient, parent or legal guardian, is responsible for any co-payments, deductibles, coinsurance, or fees not covered by insurance at the time of the appointment.

Co-payment is due at time of service unless prior arrangements have been made. We are a specialist office and co-payment may be listed under specialist on your insurance card.
